Q&A
What is the difference between a disc bulge and a herniated disc?
A disc bulge occurs when the outer layer of a spinal disc protrudes outward evenly, without breaking open. A herniated disc (also called a slipped or ruptured disc) happens when the inner gel-like material pushes through a tear in the outer layer. Both can cause similar symptoms, but a herniation typically causes more severe nerve compression and pain.
Can a disc bulge heal on its own?
Yes, in many cases, a disc bulge can improve without surgery through conservative treatment like physical therapy, rest, posture correction, and targeted exercises. Healing time varies depending on the severity of the bulge and the individual’s activity level and overall health.
When should I see a doctor about back pain from a disc bulge?
You should see a doctor if your back pain persists for more than a few weeks, worsens over time, or includes symptoms like numbness, tingling, weakness, or pain radiating into your arms or legs. These may indicate nerve involvement and require professional evaluation and care.
What types of treatment can help with a disc bulge?
Physical therapy is one of the most effective non-surgical treatments. It focuses on relieving pressure, improving mobility, and strengthening the muscles that support your spine. Other options may include anti-inflammatory medications, activity modification, posture correction, and in rare cases, injections or surgery.

Without a doubt this is the best Physical Therapy experience I have ever had (and I have had several). I have worked with Ayoub Zianai (AZ) and Hesham Abdel-Salam and they are both consummate professionals. A very thorough evaluation was done to start and a treatment plan reviewed. So much more extensive than other PT facilities which just take the referring Dr's prescription and assign exercises. Each week we discuss what is happening and modifications are made if necessary. Both practitioners are easy to speak with and very caring. I highly recommend TheraCave!Posted on Google![]()

I can’t recommend Theracave enough! I came in with herniated discs in my neck that were causing arm pain and making it difficult to work out at all. Hesham took a truly one-on-one approach, helping me understand where the pain was coming from and working on strengthening the areas of my back that needed support. He told me he’d get me back in the gym, and after about 18 sessions, I’m slowly getting back to running and lifting weights with so much more confidence. I also worked with Ayoub, who is lovely, attentive and patient. Both made me feel heard and supported throughout the process. The medical massages made a huge difference, with some of my pain going away and never returning, and the dry needling helped loosen things up. What stands out most is their gentle, science-backed and detail-oriented approach. It’s clear they understand the body and take the time to address what’s actually causing your symptoms. If you’re working toward a goal and want truly personalized care, I highly recommend them!Posted on Google![]()
